1. Supporting documents to be submitted for the over-the-counter application：
(1) The applicant: The original of the ID card should be submitted.
(2) Agent: The agent should present the original of the ID card,submit the original proxy statement or letter of authorization and the applicant's ID card; if the photocopy of the applicant’s ID card is presented by the agent, the applicant or the agent must guarantee that the photocopy is the same as the original one.
2. The various lists are provided in the unit of individuals. There is no list for the whole household. Please confirm the members of the whole household with the application (subsidy) department first. If the applicant can not apply in person, the application should be the same as the agent method (please refer to instructions 1-(2)).
